SCHOLARSHIP APPLICATION INSTRUCTIONS
Dear Scholarship Applicant,
The Iota Graduate Chapter of Omega Psi Phi Fraternity, Inc. is pleased to invite you to take part in the chapter’s Scholarship Competition. For years we have selected scholarship recipients from high schools across the Chicagoland area based on academic performance, activity in the community and personal character. We look forward to awarding more deserving students for 2017!
Eligible students are African-American high school seniors, attending schools in Chicago, with at least a 2.5 unweighted GPA, who will begin college in the fall of 2017. We urge you to carefully review the enclosed materials and submit a scholarship application along with all attachments. Our chapter is committed to identifying and awarding scholarships to deserving students who will be furthering their education by enrolling at an accredited college or university.
The application/questionnaire is designed to collect academic and personal background information for the selection process. Your response to the questions is a decisive factor in the selection process. You must answer all questions thoroughly and honestly. Failure to answer all questions and/or complete the essay portion of the application will result in immediate disqualification. You may apply online at the Academic Works website: https://cps.academicworks.com or complete the hard copy.
After completing the hard copy application, you should give the application/questionnaire to your school counselor or other designated school official to complete their section. The school official will provide the requested data and attach a sealed official transcript of grades through and including the first semester of the current academic year (2016-2017). Please mail the completed application with all the attachments to:
Iota Graduate Chapter
Attn: Scholarship Committee
P.O. Box 201266
Chicago, IL 60620
*It is the applicant’s responsibility to ensure that all information is received by April 1, 2017.
Three (3) personal references must accompany the application in a separate sealed envelope, with the recommender’s signature across the seal. Individuals who can provide information about your schoolwork, community service, and/or personal character should complete the references. Two of the references should be current teachers or school administrators. The references must be processed with the online application at the Academic Works website or mailed with the application to the address above, and cannot be sent separately.
The hard copy application is also available for download at: http://www.omegapsiphi1911.org/home.html
Only scholarship recipients will be notified by June 2, 2017. Recipients will receive their award certificates during an awards luncheon on June 23(subject to change). If you are selected as a scholarship recipient, you will be expected to confirm your final enrollment plans and begin full-time study in the fall of 2017, prior to receiving your cash award. Detailed instructions will be given to each recipient after the awards luncheon.
We welcome your application and look forward to adding your name to the roster of Iota Chapter scholarship recipients. GOOD LUCK!
